Every patient interaction leaves a question behind: what happens next? In most practices the answer lives in someone’s head, or on a sticky note at the edge of a monitor, until the end of the day. Nobody is careless. There is simply more work after each visit than any person can hold, and some of it falls through.

Smug is what finished feels like. It is the look on a front desk’s face when the parent asks for a school note and it is already printed. When a specialist calls about the referral and the answer is “sent at 11:15, with the history.” When it is 4:30 and there is nothing left to remember.
